This Root Cause Analysis template documents what happened, the impact, and the prescribed remedy, if any. It’s developed to support a wide range of project types and sizes. Specifically, this template:
- begins with a section that captures relevant project information;
- provides for documenting the event information; (E.g., it captures the reporting date, the person who reported the event, the event location, and the date upon which the root cause analysis is completed)
- prompts the project manager to describe the event in detail, describe the expected result, the impact, the persons affected, and describe the actual result and any resulting harm;
- provides for verification, preventative or corrective action, implementation, and monitoring; and
- identifies the person who will document the relevant lessons learned.
Who Creates the Root Cause Analysis?
Typically, the Project Manager is responsible for performing or leading a root cause analysis. Doing so usually accounts for the input of others.
What Are the Inputs?
There are numerous possible inputs to performing a root cause analysis, including systems documentation, test and inspection reports, and more.
