RACI Chart Template

This one-page RACI Chart template is developed by a PMP certified project manager. It is fully editable and ready for use on your project.

A RACI Chart template is used to develop an assignment matrix that identifies by task and person, those who are (r)esponsible to complete a task, (a)ccountable for approving the completed task, (c)onsulted with about the task, and  kept (i)nformed about the progress of the task. This template is developed to support a wide range of project types and sizes. Specifically, it:

  • begins with a section that captures relevant project information;
  • provides sections for identifying tasks and individuals; and
  • for each task assigned to an individual, provides a section to indicate whether the person is responsible, accountable, consulted, or informed.

Who Creates the RACI Chart?

The Project Manager often develops the RACI Chart with feedback from other project stakeholders.

What Are the Inputs?

Inputs to preparing the RACI Chart may include the project management plan, the experience and expertise of the Project Manager, and feedback from project stakeholders.
