A RACI Chart template is used to develop an assignment matrix that identifies by task and person, those who are (r)esponsible to complete a task, (a)ccountable for approving the completed task, (c)onsulted with about the task, and  kept (i)nformed about the progress of the task. This template is developed to support a wide range of project types and sizes. Specifically, it:

  • begins with a section that captures relevant project information;
  • provides sections for identifying tasks and individuals; and
  • for each task assigned to an individual, provides a section to indicate whether the person is responsible, accountable, consulted, or informed.
