This Probability and Impact Assessment template is a project document that is used evaluate the probability of a risk occurring and its’ impact on the project. It is designed to support a wide range of project types and sizes. Specifically, this template:

  • begins with a section that captures relevant project information;
  • provides sections for documenting risk topics or risk categories;
  • beneath each risk topic or category, provides sections to identify the risk, grade the probability of occurrence, and evaluate the impact on the project; (E.g., it provides for assessing the impact on cost, schedule, and quality) and
  • prompts the Project Manager to provide explanatory details, define assumptions, and indicate the priority of the risk.

Who Creates the Probability and Impact Assessment?

Often, the Project Manager performs the Probability and Impact Assessment. His or her doing so accounts for the feedback of many others, including members of the project team and other project stakeholders.

Inputs to performing a probability and impact assessment vary depending upon the identified risks. Inputs may include the risk management plan, historical information, feedback from members of the project team, and feedback from other project stakeholders, and more.
